6.Certificate from an occupational physician in case of application to medical faculties
7.Sickness or accident insurance policy for the period of education in Poland or European Health Insurance Card

Graduation diploma issued in countries other than those indicated above, not subject to nostrification

1.All documents under the heading of documents obtained in Poland
2.Certificate of proficiency in the language of study at the level of min. B2 (according to the list from the regulation of the Ministry of Education)
3.Sworn translation of all documents in a language other than Polish
4.Document confirming legalization of stay in Poland
5.Confirmation of passing the entrance exam conducted by the SAN University
6.Confirmation of payment of the first installment of tuition fees
7.Apostille or legalization
For bachelor's and master's degrees, confirmation that the certificate entitles the student to study (nostrification):
